Have you been naturalised? Have you been recognised as a refugee in Germany?

The following declarations can be made:

• Declarations on the first name and surname

• discard parts of the name that are not provided for under German law (e.g. patronymic)

• adopt the original form of a name modified according to gender or family relationship
